Dr. Sheffield is a prominent Santa Barbara plastic surgeon who specializes in injectable fillers and non-surgical facial rejuvenation. Some of our patients have brought forth interesting and detailed questions about dermal fillers, otherwise known as cosmetic fillers of facial fillers.

: How Much Bruising Should I Expect from Under Eye Filler Injections?

How bad is the bruising under the eye area after filler injections are done? Do I need to take time off work?

: Thank you for that question. Bruising is a possible after effect which you could get from undergoing filler treatments. Actually, bruising is a possible after effect of any kind of injection especially if it is done under the eyes. The main reason for this is that the skin under the eyes is quite thinner compared to other parts of the body. Fortunately, bruising can still be avoided, if you do proper after treatment care as well as if you select an experienced and skilled injector such as a board certified plastic surgeon. The skill of the injector is very important as your injector can avert or prevent possible after effects if he is experienced enough.

Related: ASK DR. SHEFFIELD – WHAT IS THE BEST DERMAL FILLER FOR CHIN/JAW AUGMENTATION?

For after care maintenance, you can ice the injection site after the treatment. However, do not apply the ice directly on your skin to avoid burns. In addition, avoid products that can thin your blood prior and after the procedure so that bruising may be prevented. Such medications would include aspiring, Coumadin, vitamin E, anti-inflammatory medications, and the likes.

Bruising will usually subside on its own. It may last anywhere within 24 to 48 hours or a max of 7 to 10 days. If bruising persists longer, you may want to contact your surgeon for a follow up checkup. Hope this answers your question!

What is the Best Dermal Filler for Chin/jaw Augmentation?

I have been considering a chin/jaw augmentation and wanted to know what are the best/longest lasting fillers to use. Also can all the dermal fillers be applied thru a microdroplet technique(like silikon 1000), or are some fillers less precise and have to be applied a different way, thanks

: Thank you for your interest. If you are looking to have a jaw or chin augmentation, fillers can be the perfect solution. Volume in the chin or jaw area can be sustained by using hyaluronic acid dermal fillers. One hyaluronic acid dermal filler that can be used would be Restylane. Restylane gives a natural looking result as well as exude a smooth and feminine like appearance. Aside from Restylane, you may also use Juvederm Voluma. Juvederm Voluma is considered as a volumizing produce having a higher density. It also has a soft result while contouring the area perfectly. The result also looks natural.

: Can Injectable Fillers Be Used on Crow’s Feet?

: Thank you for your inquiry. Yes, dermal fillers can be used for improving crow’s feet and there are a number of fillers, which can do the job. In my practice, I usually make use of Juvederm or Restylane. Others may also have Belotero and Teosyal in their list. All these fillers can help in smoothening out the fine lines and wrinkles that are observable even when your face is relaxed. Other than fillers, you can also opt for Botox to soften up Crow’s Feet that appear when you smile. At times, Botox and fillers are used to together to address Crow’s Feet that appear when you are smiling and when your face is relaxed to achieve a more youthful appearance.

: How Do I Get Rid of Wrinkle Filler if I Don’t Like the Results?

I read that injectables can be extracted from the skin if you don’t like the results.  How is this possible?  Laverné from Indianapolis, IN

: That is a good question. Yes, it is possible to remove some fillers once they are already injected in your face. Dermal fillers that have hyaluronic acid base such as Restylane, Juvederm, Perlane, and Teosyal can be removed by dissolving them using an enzyme called Hyaluronidase. This is the reason why many patients and doctors use hyaluronic acid dermal fillers – they are very easy to dissolve should there be an issue or if the patient is not satisfied with the result. On the other hand, permanent fillers like collagen filler, can be removed by excising them surgically. Other fillers like Radiesse, Artefill, and Sculptra do not contain hyaluronic acid. They are composed of various substances making the reversal quite difficult and challenging to perform. Extracting the fillers would be the only option available for these fillers. Unfortunately, even with the option to extract, success is still not guaranteed, as extraction may sometimes not work out well.

I would highly recommend a thorough discussing with your board certified plastic surgeon prior to getting a filler treatment. Discuss your options and open up the possibility of getting a reversal in case you are unhappy with the result. Best of luck!

Eyelid Surgery Santa Barbara

Dr. Sheffield is a prominent Santa Barbara plastic surgeon who specializes in eyelid surgery and non-surgical facial rejuvenation. Some of our patients have brought forth interesting and detailed questions about upper eyelid surgery, otherwise known as upper blepharoplasty, and eye bags treatment.

: Fillers for Under Eye Hollowness and Dark Circles?

I’m a 27 year old model. Since my teenage years I’ve suffered from under eye tear throughs problem. I also have some puffiness in the lower eyelid which doesn’t bother me. The hollowness doesn’t bother me either.

What bothers me is the blue vein you clearly see which give me persistent dark circles. My skin is really thin but I know there is no skin discoloration. When I scrunch my skin together, the dark circle disappears. What I want is just a minimal filler (buffer) between my skin and the vein to get rid of the blue shade. Am I a good candidate?

: Good question and thank you for the photo that you attached! Based on what I have observed from the photo that you have attached, the dark circles may actually be brought about the hollowness from your under eye. A dermal filler may be injected in the area where there is hollowness. Once this under eye hollowness is improved, your dark circles will be reduced significantly as well. Teosyal may be used for this case since this filler has nutrients that can help in minimizing discoloration.

You may also benefit from other fillers such as hyaluronic acid fillers. Such fillers can provide a smoother and more natural result compared to other fillers. Injecting some in the hollowed under eye can help add volume to it. Hyaluronic acid fillers are awesome since they are easily reversible and very easy to use. They also have very little downtime.

: How Do I Get Rid of Eye Bags?

My under eye bags make me look tired at all times. Is eyelid surgery the only way I can get rid of eye bags!

: Thank you for that question. Eye bags appear when facial volume is lost and fat pads show up in the lower lids. Previously, the only method to improve or remove eye bags is through blepharoplasty. Blepharoplasty is a kind of surgery that repairs droopy eyelids. This surgery works by removing the extra fat in the lower eyelid area.

Having said that, technology and science has progressed immensely over the years and the process of improving eye bags has also improved. Instead of removing the excess fat in the lower eyelid, this fat is preserved and lost volume is replaced. This process involves non-surgical methods such as dermal filler injections. Restylane is one dermal filler, which should resolve eye bags. Moreover, only a small amount of this filler is needed to regain a well-rested and youthful appearance of the eyes. Filler injection treatment usually lasts for roughly 15 minutes. It is painless and the results show up almost immediately. With a good injector, filler injection is deemed effective and safe.

So, if you are interested in having your eye bags taken care of, consult a board certified plastic surgeon who has already performed filler injections. Inquire about the results of their previous treatment so you can be sure that you are dealing with a skilled injector. Hope this answers your question!

:How to Repairs my Hallow/sunken Eyes?

My eyes are so hollowed. i’ve turn 20 and look like in my 40s. i hate this look. i’ve got exam coming up and i can’t focus on it due to my look. i am so embarrassed to go outside or even face my friend. i hate my face. people tell me to be happy but i can’t even after trying so hard because i keep worrying. i tried all home rem ides to solve it. i do have 7.5 to 8 hours sleep. i drink lots water. i think this could be genetic but i really want way out of this problem. please give me some advice.

: Thank you for your inquiry and for posting an image for us to reference to. About your concern, you mentioned that you want to have your sunken eyes improved so that the hollowness is repaired. To start off, we need to get an idea as to what the root of the issue is. Hollowness or sunken eyes may be caused by a number of factors. You are right, it could be genetics, but it could also be lifestyle, age, and environment. That said, your attending surgeon, may need to assess you properly to identify the best treatment for your sunken eyes and hollowness.

Related: ASK DR. SHEFFIELD – IS THERE A NONSURGICAL WAY TO REMOVE BAGS FROM UNDER MY EYES?

Based on the photo that you have provided, I can tell that there is some fat loss on both the upper lid and the lower lid. This fat loss is the reason for the shadowing and irregular contour on the area. Moreover, the purplish hue of the skin surrounding the lids may possibly be caused by heredity. However, I cannot tell for sure since I am only looking at a photo. Generally speaking, there are possible treatment methods that could be used to treat hollowness in the eye area. My preferred treatment method for this would be via dermal fillers such as Juvederm or Restylane. These are hyaluronic acid fillers that can help add volume that will result to a natural and smooth skin. Once the hollowness is filled, the dark discoloration may lighten up a bit. It can then be managed using other treatment methods for lightening the skin. Hope this is helpful!

Is There a Nonsurgical Way to Remove Bags from Under my Eyes?

I am 52 years old and I have bags under my eyes. One is worse than the other and I would like to know a nonsurgical way to get them to go away. I don’t have any winkles and this is the only thing that gives my age away. Also being on a fixed income I would like to know how I can get rid of them the most cheapest way possible. Can anyone advise of a nonsurgical solution?

: Thank you for your interest on the matter! Actually, there are no non-surgical means that could remove eye bags. Surgery like blepharoplasty is still the way to go if your goal is to “remove” your eye bags. However, there ARE non-surgical methods, which may help improve, cover, or reduce the eye bags even by a bit.

But before anything else, you need to know that the severity of the fat herniation will tell if the non-surgical means of improving eye bags will help you achieve your desired result. This will be determined when you visit a board certified plastic surgeon in person for an eye bag evaluation. If your surgeon finds that your eye bags are not very prominent, non-surgical means like filler injections could be done in order to blend the eye bags into your cheeks. Getting fillers can help conceal or mask the eye bags for about 1 year.

Related: ASK DR. SHEFFIELD – WHAT CAUSES WATER RETENTION UNDER MY EYES?

That being said, always consult a board certified plastic surgeon so that he or she can evaluate your case as well as consider the best options for you. Your surgeon will consider all of the possible causes of the eye bags and the factors that could help identify the best non-surgical option that give you your required result. I hope you find this helpful!

:What Causes Water Retention Under my Eyes?

I am 47 years old and I have developed malar edema. It sits right above my cheekbones and will not go away. Should I find out if there is an underlying health condition before I go to a plastic surgeon and if so, what type of tests should be done? Thanks

:That is a good question and I would like to take this opportunity to give clarification to the said “water retention” under the eyes issue. Malar edema is more than just about water retention issues, this is in reality “orbital fat prolapse.” Orbital fat prolapse is characterized by a movable mass under the conjunctiva. It is often linked with localized thinning or weakness of the Tenon’s capsule. This phenomenon may be brought about by surgical trauma or by natural means such as aging. This may be resolved via blepharoplasty or autologous fat transfer, but some improvement may also be seen through a non-surgical approach of using fillers. Under the right and skillful hands of an experienced injector, this should be resolved effectively.

Related: Ask Dr. Sheffield – Is There a Nonsurgical Way to Remove Bags from Under my Eyes?

That said, it would be helpful if you seek in-person consultation with your board certified plastic surgeon so he or she could assess the malar edema. From here on, he or she will be able to analyze and develop a treatment plan to resolve it. Best of luck!

doctor feels fairly confident that there will be minimal scarring and the shape of my eye will not change. My fear is, if the shape of my eye is changed, can it be corrected?I am nervous about the procedure and skeptical that valium and local anesthesia wld b enough to calm my nerves and have been offered propofol (drug that killed micheal jackson)Is it ok to use this drug for such a short and minor procedure? I had propofol before with no problems for a colonoscopy.

:Good question! By your description, I am guessing the yellow cholesterol deposit on the under eye area may be xanthelasma. If this is xanthelasma, it should be painless to treat and quite easy to work on as well. You may need to be numb for this so an injection of Lidocaine may be used on the area. Should you be anxious, valium may work to calm your nerves. The after effects of the treatment for xanthelasma may include minimal scarring and/or bruising. Regarding the eyelid shape, if the treatment is done properly, the shape should stay the same.
